Child Day Care Mission Statement
FACCDC may enroll up to 50 children.
Before a child can be admitted an enrollment package has to be completed which includes the folowing:
- Completed Application
- Discipline Statement
- Child's Health & Emergency Information
- Fee Agreement
- $25.00 Non-Refundable Registration Fee
- Shot Record
- Birth Certificate
- Medical Report

We encourage all parents to participate in our open door policy. This helps make your child's first experience at the center a pleasant one. This also gives our staff an opportunity to get acquired with you. So please come in at any time.
